Change of name and/or address for premises licence holder or DPS
If you wish to change the name and/or address of the premises licence holder or designated premises supervisor please use the notification form below.
Please note this is only for circumstances where the premises licence is held by the same person or company but they have changed their name, for example, by marriage.
If a new person or company has taken on responsibility for the licence, then an application to transfer the premises licence is required.

Fees
The fee for an amended premises licence is £10.50.
Attachments required
The full existing premises licence including the summary must be returned along with the form. If you are unable to return the licence then you must state a reason why.
