Health and Wellbeing Partnerships
Who are we?
The Purpose of the Health and Wellbeing Partnerships is to develop strategies and support activities for Broadland and South Norfolk that; Promote good health and wellbeing of communities, address the wider determinants of poor health, tackle health inequalities, develop and deploy support which prevents crises, align, develop and influence health and wellbeing services and commissioning and support people to live well in their own homes
To do this we are focused on the following purposes:
- Be bold, confident and transparent in our decision making
- Use local intelligence, listening to local voices and using evidence to set priorities
- Focus on the person, not the system
- Work collaboratively and proactively for our communities
- Embrace trust, honesty and kindness between all partners
- Innovate, test, review and share learning regularly.
Priority Delivery Groups (PDG)
Priority Delivery groups meet up to support the development of proposals and drive forward the work that delivers our three main themes. The group reviews national strategies and policies to ensure our work is aligned, we do this with the help of subject matter experts. Each of our Priority Delivery Groups develop and agree proposals that meet the Action Statements set out our strategies, outlined below.
Access, integration and prevention
It is important that access to services is equitable and that we are supporting people to live well and preventing or delaying the need for more formal interventions.
Action statements:
- We will work collaboratively to identify individuals who may require additional support and ensure they are connected with the appropriate services and resources to maintain or improve their health and wellbeing.
- We will look to understand and support the opportunities for people with musculoskeletal conditions to improve and manage their condition to and work to prevent these conditions for those who are at risk.
- We will work together to explore sharing data and intelligence to better target individuals and families in need of help.
- By prioritising falls prevention, we aim to delay or reduce the need for care, supporting independence and enabling people to live well at home for longer.
Projects include:
- Proactive Interventions: To identify those at risk of a fall using AI then being able to organise and conduct a person-centred strengths based assessment, covering all areas that may affect wellbeing and health. Based on this conversation the team can then work with the customer to priorities their support needs and identify the best form of intervention.
- PROSPER (Proactive Social Prescribing, Extended Reach): Targeted support for families that are in poverty to maximise income, improve engagement with health services, improve engagement with local communities and engage with other agencies as required.
- Adult Social Care Integration: Bringing together district and adult social care services to create integrated, jointly resourced and person-centred support
- Neighbourhood Networking Event: a series of in-person networking events that bring together frontline colleagues from across public services and the voluntary and community sector to combine knowledge and resource.
Supporting good mental health
We recognise that mental health is ever shifting and is influenced by a number of factors. We seek to equip people with the skills and tools to recognise when their, or someone else’s, mental health is declining in order to signpost and support good mental health.
Action statements:
- We will support and promote good mental health across the district, taking a holistic approach to wellbeing and supporting the development of a Mindful Norfolk approach.
- We will work to identify those who might benefit from a more targeted approach to improve their mental health and will develop projects to meet those needs.
- We will ensure there is access to clear, joined-up information to empower people to understand and improve their own mental health and wellbeing.
- We will collaborate with our partners to increase awareness of dementia, risk factors, and help delay its onset.
Projects include:
- Cup-O-T: Deliver preventative mental health occupational therapy groups and provide complementary training to referrers in Broadland and South Norfolk.
- Prevention work with construction and agriculture workers: Targeted work with construction and agriculture workers to understand and offer mental health support.
- Mindful Norfolk - Creation of a county wide brand supporting mental health.
- Mindful Towns and Villages
Developing resilient and healthy communities
By asking and listening to our communities, we will support and develop environments where individuals and communities can thrive through improved health, wellbeing, and social connection.
Action statements:
- We will support the development of healthy places, promoting access to parks, green spaces, and local leisure and community facilities.
- We will encourage people to be, and remain, active to maintain their health, wellbeing and mobility.
- To understand and address the barriers that people face when it comes to accessing food and making healthier food choices.
- We will work closely with our communities to identify their strengths and help them to build connections and resilience to ensure support is available in their own neighbourhoods for the health and wellbeing priorities that they identify.
- We will support and encourage initiatives that enable communities and people to come together, promoting the positive impact of social connection.
- We will work to develop and support initiatives that prioritise ensuring homes are of a good standard and meet the needs of those living in them.
Projects include:
- Movement and Nutrition: Working with communities to understand low movement and poor nutrition, then providing a targeted approach to combat the problem.
- Clear Company: Community Cooking for Wellbeing delivers hands-on, participatory cookery workshops that bring people together to learn affordable cooking skills, share meals, and build social connections.
- Social Moments: Assistance with taking on a new staff member to coordinate existing and potential activities relating to social isolation, children and young people, diet and nutrition.
- Access to Equipment: To provide access to sports equipment where deprivation and poverty are a barrier to accessing spaces.
Cross-cutting actions
These action statements sit across all our projects, and we look to address these through all our work
Action statements:
- We will continue working together to explore ways of sharing data and intelligence to better identify and support individuals and families in need.
- Our approach will prioritise prevention and interventions which will allow people to live well for longer.
- To address health inequalities, we will support individuals, ensuring they can access the resources and services they need to improve their health and wellbeing.
- We will promote services that assist individuals in joining or re-entering the workforce and taking up volunteering opportunities.
- We will identify and work collaboratively to pursue funding opportunities to support joint efforts in reducing health inequalities and which embed a preventative approach to service delivery.
- Our approach will be underpinned by co-production, engaging people to share their insights and help shape our work based on their lived experience.
- We will continue to work to further integrate our services putting the person at the centre of our ways of working.
Steering Leadership Group (SLG)
Our Steering Leadership Group receives updates on projects and proposals to provide constructive feedback, providing steer on risks and issues and giving direction to PDGs. Members of our SLG are senior stakeholders from our partners organisations.
Health and Wellbeing Partnership (HWP)
Chaired by the Portfolio Holder on behalf of the Council, the HWP has oversight of the whole programme of work, agreeing direction and priorities and approving larger funding proposals.
