Lost personal licence

If you have lost your personal licence please contact us in writing via email or letter for a replacement.

Change of name and/or address

You must apply to the local council in the area where your licence was originally granted and not to the council in which you now reside or the licensed premises is located.

Documents required

If you are notifying us of a change to your name and/or address, both the A4 paper and plastic card licences must be returned along with the form. If you are unable to return either of these licences then you must state a reason why.

The fee for any replacement personal licence is £10.50
